What to Do
Please plan to arrive 15 mins prior to you scheduled appointment if you haven’t filled out the new patient form, and plan to spend 60 to 75 mins in the office. During this time you will receive a comprehensive oral examination and x-rays to help your dentist access your over all oral health. Dentist will look for signs of:
- Tooth Decay
- Bone recession
- Periodontal diseases
- Teeth deterioration: abonormal bite, bruxism, or TMJ
- Mal occlussion
- Oral Cancer

To get started we would need information about you. Please download and complete the forms and bring it with you for your first visit.
What to Bring
Please bring with you the following:
- Dental insurance card, if applicable
- Photo ID
- Completed new patient form
- List of any questions or concerns you may have
